In Case of Emergency:
Please call 911 and go to the nearest emergency room if you have severe chest pains, shortness of breath or any other life-threatening symptoms.
Appointments:
Our offices are open from 8 a.m to 5 p.m., Monday through Friday. The office will be closed for a brief lunch break between 12 noon- 1:00 p.m. Our answering will be available to take any messages during this time. Patients are seen by appointment only. Your first appointment may take up to one hour. In case of patient emergencies our doctors may run late or need to leave early, we ask for your patience and understanding on these rare occasions.
Please bring the following to your appointment:
- For first appointments, your paperwork that can be downloaded here
- Drivers license & Insurance cards
- All medications in their original containers with the labels (if possible) if not a list of all medications with dosage information
- Primary care physician authorization
- Co-payment
- If an HMO - referral authorization
Medications:
If possible, please bring all your medication in their original containers for every appointment and request refills and the time of your appointment. For telephone refill requests, please call during business hours and have the medication name, dosage and pharmacy number readily available to provide our staff.
Cancellations:
We ask you make any cancellations a minimum of 24 hours prior to your appointment, or it may result in a $25.00 office visit charge.

Sunshine for Seniors – helps eligible seniors (age 60 and older) or disabled Medicare beneficiaries take advantage of prescription assistance through existing pharmaceutical patient assistance and retail discount programs. These programs offer prescription drugs at reduced prices and in some cases at no cost to eligible individuals. The program also helps them locate a pharmaceutical assistance program, and when needed assists with the application process.

Community Health Care Centers / Rurual Health Clinics – located statewide and accept Medicare assignment. The annual Medicare deductible is usually waived. Income eligible seniors receive services for free or on a sliding scale.
Rurual Health Clinics – offer basically the same benefits with exception that the yearly deductible is not waived
Medicaid Assistance – some Medicaid programs help pay for premiums, deductibles and coinsurance for certain people who have a limited income and are entitled to Medicare
Medicare Prescription Drug Coverage – assist with obtaining prescription drug coverage as required by Medicare. If you have a limited income, you may also be eligible for extra-help saving on plan premiums, deductibles, and co-payments.
Free Assistance for the Uninsured – program options provide drugs at reduced prices, or in some cases, at no cost to eligible individuals.
Military Programs – Veterans benefits are available to individuals who have served in the U.S. Military with an honorable discharge. Prescription drugs are dispensed at a cost of $8.00 per prescription.
Tricare – offers a health care plan for personnel who have retired from the Army, Navy, and Air Force. Senior pharmacy benefits are included in their coverage. Individuals must be eligible for Medicare Part A, enrolled in Part B, and registered in DEERS.
